Application Forms

The Application Form is to be completed and signed by you and returned to Friends Meeting School with a non-refundable application fee.

Complete and return this form to register your child for BeforeCare, AfterCare, or our Falcon Shuttle!

The Recommendation Form is to be completed by your child's teacher or school administrator. If your child is not in school, the form should be completed by an adult who has worked with your child in a group setting.

Note: A second recommendation form should be filled out for students entering grades 7-12.

The Student Information Form should be sent to your child's school for completion if your child receives a report card or a progress report.
